ATHLETIC FEE
The $50 athletic fee is due before athletes can participate in practice or competition. All athletic fees should be paid in the Athletic Office of the High School. Checks should be made out to Pewaukee High School. Checks submitted for sports that require try-outs are held until the teams are formed in the event the check needs to be returned.
The Athletic fee is the only thing that should be turned into the office for registration. All other forms should be completed during the online registration process.
ATHLETIC FORMS
The following documents (5) plus the athletic fee must be on file in the athletic office before student-athletes can begin participating in practices or competitions. All forms should be accepted and signed electronically through the online registration process. The only paper component of the entire process is the uploading of your current Physical Card or Alternate Year Card. You do NOT need to submit a paper copy of the Physical Form or Alternate Year Card to the Athletic Office. Keep on file for yourself for future use.
The following information will outline what is needed to register for athletic participation at Pewaukee High School. All athletes are required to have the required information and signatures on file prior to participating in any capacity. This includes practices, scrimmages, and competition.
Current Physical
Athletes are required to have sports physicals on file with the Athletic Department prior to the first practice. Failure to do this results in ineligibility to participate in try-outs, practices or games.
Sports physicals (green cards) are required by the WIAA every two school years. Physicals taken after April 1 are valid for the following two school years; physicals taken before April 1 are valid only for the remainder of that school year and the following school year. In the second school year that the physical is valid, a tan alternate year card is required. This updates personal insurance and physician information and gives permission for the student to participate. The tan card only requires a parent’s signature.
The Athletic Department does not work in conjunction with youth sports, camps, etc. so DO NOT assume we have copies of physicals provided for those purposes. Athletic Code
It is a privilege to represent the Pirates in competition. All student-athletes are required to sign the Pewaukee High School Athletic & Activities Code prior to participating with our programs. The required form can be downloaded or picked up in the athletic office at PHS. Please review the code with your students and be sure all the expectations and consequences are clearly understood. The contract with student and guardian signatures must be on file at Pewaukee High School prior to participating with our programs. Please call with any questions.
Random Drug Testing Consent Form
The Pewaukee Board of Education has a strong commitment to the health, safety and welfare of all students. Given this fact, the Pewaukee School District conducts a random drug testing program for all students involved in athletics and activities. Students, by virtue of their voluntary decision to participate in these activities, and because of their position as school leaders and role models in their school community, have a heightened responsibility to be drug and alcohol free.
Concussion Info Acknowledgement
State mandated form required from each athlete once per school year. To be signed by parent and athlete before 1st practice.
WIAA Eligibility Information
All athletes must meet the eligibility requirements established by the Wisconsin Interscholastic Athletic Association in order to compete for Pewaukee High School. Additionally, athletes and parents are required to sign a form indicating their understanding and compliance with the rules of eligibility established by the WIAA. The rules and required form can be downloaded or picked up in the athletic & activities office at Pewaukee HS.
ADDITIONAL FORMS
Athletic/Activity Code Appeal Request Form
This form is used to appeal the penalties of a code violation.
Alternate Transportation
This form must be completed and handed in to the coach for alternate transportation to or from a sporting event.
